Risk Detection Engine

Medical Core

Before we build a plan, we build a picture. The Medical Core is the deep, structured baseline assessment of the systems most predictive of long-term health: cardiometabolic, hormonal, metabolic, and structural risk.

What we test

  • Genetics testing — know your predispositions, drug response, and biology
  • Functional labs — ApoB, insulin, hs-CRP, Omega 3:6 (6 levels), inflammation, and more
  • Hormone assessment & treatment — comprehensive hormone panel and optimization as needed
  • Gut health — GI Effects stool test for gut health and inflammation
  • VO₂ Max testing (home-based) & lactate testing — cardiorespiratory fitness, endurance, and metabolic threshold, performed in the comfort of your home
  • DEXA scan — body composition, muscle, fat, and bone density
  • Metabolic age assessment — know your true metabolic age
  • CGM (continuous glucose monitor) — real-time glucose and insulin response
  • Oral pathogen testing — saliva-based screening for periodontal pathogens that drive systemic inflammation
  • Case-by-case advanced imaging — Coronary Calcium Score (CAC) or CTA Coronary, when clinically indicated

Why genetic testing matters

"We don't test genetics to predict disease — we use it to personalize your path to optimal performance, longevity, and resilience."

Your DNA doesn't dictate your future, but it does explain why some people thrive on a low-carb diet while others need more carbs to perform, why one person responds to statins and another doesn't, and why two people with the same lifestyle can have wildly different cardiovascular risk. We use genetic insights to tailor every recommendation to your biology — not run a generic protocol.

Examples of what we look at

  • APOE genotype — modulates Alzheimer's, lipid metabolism, and dietary fat response
  • MTHFR — folate metabolism, methylation, and B-vitamin needs
  • Pharmacogenomics — how you metabolize statins, beta-blockers, antidepressants, common antibiotics
  • Lipid & cardiovascular SNPs — Lp(a), LPA, PCSK9 — refining your cardiovascular risk beyond standard panels
  • Nutrient absorption & macronutrient response — caffeine clearance, vitamin D conversion, lactose, alcohol metabolism
  • Exercise response genes — power vs. endurance bias, recovery rate, injury susceptibility

Why gut health matters

Roughly 70% of your immune system lives in the gut. The microbiome regulates inflammation, controls insulin sensitivity, produces neurotransmitters, and gates how much of every nutrient you actually absorb. When the gut is dysfunctional, the rest of the body pays — and standard "everything looks normal" labs miss it almost every time.

What gut dysfunction can drive

  • Leaky gut → systemic inflammation — fueling cardiovascular, autoimmune, and metabolic disease
  • Dysbiosis → insulin resistance & weight gain — the wrong bacteria can drive cravings and blunt fat loss
  • SIBO — bloating, gas, brain fog, post-meal fatigue, nutrient deficiencies
  • Gut-brain axis — anxiety, low mood, and poor sleep often have gut origins
  • Hormone clearance — gut bacteria recycle estrogen and other hormones; imbalance disrupts whole-body hormone health
  • Nutrient deficiencies — B12, iron, magnesium, fat-soluble vitamins all depend on a healthy gut

We use the GI Effects comprehensive stool test to map your microbiome, identify pathogens, measure inflammation markers, and check digestive function — so we know exactly what to fix.

Why oral pathogen testing matters

The mouth is the gateway to the bloodstream — and certain bacteria living quietly in your gums don't stay there. They translocate into the circulation with every meal, every floss, every dental procedure, and they fuel chronic inflammation in places far from the mouth.

The most-studied culprit is Porphyromonas gingivalis:

  • Heart disease — its DNA has been recovered from atherosclerotic plaque; it accelerates plaque growth and destabilizes lesions
  • Cognitive decline & Alzheimer'sP. gingivalis and its toxins (gingipains) have been identified in the brains of Alzheimer's patients, where they appear to drive neurodegeneration
  • Systemic inflammation — chronic low-grade inflammation that elevates hs-CRP, worsens insulin resistance, and accelerates biological aging
  • Other links — rheumatoid arthritis, certain cancers, adverse pregnancy outcomes, and metabolic dysfunction

A simple saliva-based oral pathogen panel reveals exactly which pathogens are present and at what burden — which allows for targeted dental intervention (deep cleaning, periodontal therapy) and medical intervention (anti-inflammatory and microbiome support) instead of guessing. It's one of the highest-leverage tests in longevity medicine and one most patients have never heard of.

Why VO₂ Max is the most important number you've never measured

VO₂ Max is the maximum amount of oxygen your body can deliver and use during peak exertion. It integrates your heart, lungs, blood, mitochondria, and skeletal muscle — every system that determines how well you age.

It is the strongest known predictor of all-cause mortality — a stronger independent risk factor than smoking, type 2 diabetes, hypertension, or coronary artery disease. Large cohort studies (including the Cleveland Clinic's 122,000-patient analysis published in JAMA Network Open) show:

  • Adults in the lowest fitness category have ~5× the mortality risk of those in the elite category over a 10-year follow-up.
  • Each 1-MET (~3.5 mL/kg/min) improvement in VO₂ Max lowers all-cause mortality risk by ~12–15%.
  • The mortality benefit of going from "low" to "below average" fitness is greater than quitting smoking.

Combined with lactate threshold testing, we issue a personalized exercise zone prescription — your exact heart-rate ranges for fat-burning Zone 2, threshold work, and VO₂ Max intervals — so every workout produces the maximum fitness gain at the lowest injury risk. We retest twice during the program so you can watch the number move.

Output: Identify early risk, metabolic dysfunction, and the 3–4 priority targets that will move your numbers most.
02
Response Tracking

Physiology Layer

Diagnostics tell us where you are. Tracking tells us whether the plan is working. The Physiology Layer is how we keep the loop tight between an intervention and the body's response — so we can adjust before months are wasted.

What we track

  • PRYSM scan — oxidative stress and antioxidant status
  • Biomarker & lifestyle tracking — sleep, HRV, recovery, activity, strain — integrated wearable data without brand restriction
  • Cognitive assessment — comprehensive testing for memory, attention, processing speed, and executive function. Reassessed at the end of the program to measure improvement.
  • Telomere length assessment (SpectraCell) — biological aging measured at the cellular level. Reassessed at the end of the program to measure improvement.
  • Continuous review by Dr. Yeruva and your dietitian
Output: Real-time visibility into biological response, recovery, and adherence — so the plan keeps adapting to you.
03
Functional Foundation

Movement & Posture

You can't optimize a body you can't move well in. The Movement & Posture layer addresses the mechanical foundation that everything else stands on.

We assess

  • Posture analysis — head, shoulders, spine, pelvis
  • Movement dysfunction and muscle imbalances
  • Mobility, stability, and flexibility
  • Spinal alignment and joint function

You receive

  • ONMM evaluation (referral-based) — Osteopathic Neuromusculoskeletal Medicine assessment via specialist referral, when clinically indicated
  • Corrective exercise prescription
  • Posture correction & ergonomic guidance
  • Mobility and movement restoration
Output: Better posture, less pain, improved movement and exercise capacity — the foundation that makes every other intervention work.
04
Sustain & Optimize

Recovery & Adherence

Most plans fail not because the science is wrong but because real life gets in the way. The Recovery & Adherence layer is the support system that makes the rest of the program stick.

What's included

  • Targeted supplements & prescription medications — when the data calls for it, your plan includes evidence-based supplements (omega-3, vitamin D, magnesium, methylated B-vitamins, etc.) and prescription medications (lipid-lowering agents, GLP-1s, hormone optimization, blood-pressure agents) chosen and dosed for your biology
  • Red light therapy mat — recovery, inflammation, mitochondrial support
  • Coaching — nutrition guidance and accountability
  • Training support — personalized exercise and program coaching
  • Lifestyle optimization — sleep, stress, sunlight, supplements, community
  • Hyperbaric oxygen therapy (referral) — for elevated inflammation, recovery support, and healing
Output: Consistency, adherence, and long-term lifestyle change — the difference between a 6-month win and a 20-year trajectory.
